What is the experience of a person who is informed that he has a malignant tumor? Fear? Annoyance? Paralysis of thoughts and actions? Probably, in a word, then a shock. When I asked the doctor of the oncourological department of one of the hospitals in Novosibirsk: “why don’t you tell the patients the truth about their diseases,” she said: “What if they start to be thrown out of the windows?” Indeed, there are cases when people rush under the train after they find out that they have cancer. This means that the word “Cancer” is today perceived even by doctors as a sentence, and this sentence without any amnesty and appeal. But all this is a profound error or a deliberate policy of the international medical industry aimed at billions of dollars in profits. The fact that cancer is curable, the author of these lines experienced first hand.

It all started, of course, long before the moment when I, like many deceased and cured, recognized the “terrible” diagnosis of CANCER.

Therefore, you can understand my condition when, on December 26, 2012, I underwent an ultrasound and the doctor who did the analysis told me that I have a large tumor in my bladder. Going home, I was still hoping inwardly, and suddenly she was benign. And for me it would be good news. But when I got home and looked on the Internet, I realized that my hopes were not destined to come true, since there are practically no benign bladder tumors. And going the next day to the urologist of a private clinic, I already in black imagined my prospect. But at the same time, even in the evening of the previous day, having plunged into medical terms and studied a number of articles on bladder cancer, I already realized that it would be nice if I had cancer of the second stage, then I still had the chance to stop this process with known medical means, and God would give - subsequently and recover.

At the previous stage of my life, I always adhered to the principle: if I can’t influence some events, then there is no reason to suffer, worry and regret, and if I can, then I’ll do my best to make my influence as much as possible direction.And due to my illness, I also made the decision: I’ll act, because I still have so many plans for life, but when I once planned to live at least 300 years. But, and most importantly, I have to break the trend that has begun to take shape in my family. But this trend is this: my father Lobarev Vasily Ivanovich died at the age of 56 from stomach cancer, my paternal grandmother Matrena Stepanovna Lobareva, at 79, also died from stomach cancer. And so that my children would not be programmed for this trend, I must change it. And my further deepening in this topic showed that this is the only right way. Neither any gloom, nor any fear, only the focus on victory is one, and in rare cases the only way to cure. I will give a real example from the literature of the influence of psychological factors on healing. One citizen was diagnosed with stage 4 cancer and 4 months left to live, well, a maximum of 6 months. What did this citizen do? He decided to live these remaining months to his full pleasure. So the next day, December 27, 2012, I went to an appointment with a urologist. He talked with me and heard that an ultrasound scan of my bladder was accidentally detected yesterday. What, in general, did not seem to be any symptoms. But then, I remembered that in the spring I had discomfort in the area of ​​the prostate, and in the summer after intense cycling several times the urine was just brown. After analyzing all my testimonies, he made the assumption that my tumor has been developing for about 1-2 years. And he began to “scold” me that with any fact of the appearance of blood in the urine, it would be necessary to immediately run to the urologist. I was interested in the question: why did I, a non-smoker and a non-drinker, start to develop cancer. The urologist explained that the cause could be long work in the chemical industry, stress, hypothermia and other causes that led to a weakening the immune system.

What could be the reason? Yes, the whole of 2011, as the head of the organizing committee “Pensioners for a decent life,” I had to freeze at rallies that we held every two weeks in spite of the frost and heat. Yes, twice I had to spend two and three days in OVS in the same cell with drunks and drug addicts. Yes, they burnt my car, they beat me and took me to the forest, so that I would stop organizing rallies for the return of unlimited travel, canceled by the governor of the NSO, Yurchenko V.A. At first the police, and then the policemen, grabbed me at almost all the rallies and dragged me to the courts, allegedly for violating the order of rallies. Could this affect and cause stress and cancer? Yes, probably it could. At the same time, there falls the time of “problems” with my eldest son, Pavel, and the time of solving the issues of its adaptation. Although I have a hardened nervous system, this could be the reason. Yes, all this could fail in the immune system and lead to cancer. Today, everyone in Russia has enough reasons that can affect the weakening of immunity. Not without reason in our country cancer mortality is slightly inferior to mortality from cardiovascular diseases, taking the second place.

The doctor explained that first you need to take a biopsy, that is, pinch off part of the tumor to determine its characteristics, its aggressiveness. A biopsy is an operation.It is done as follows: through a catheter, which is inserted into the urethra into the bladder, the surgical instrument penetrates the bladder together with the camera, and tissue samples from the tumor are already taken for analysis using the inserted surgical instrument. He also explained that in oncology, the main thing is not to waste time, and the faster I do it, the better the prospect of treatment can be. But they do not have such operations in the clinic (168 medical units). In addition, he told me that if you do a biopsy, it is better to immediately cut out that part of the tumor that has grown into the inside of the bladder. Based on the knowledge that I gained digging through on the Internet before, I said that I agree to do this operation, which is called TUR (transurethral resection), a bladder biopsy. Such operations are not always done. I said that let him tell me where to turn in order to do this as soon as possible. He gave me several addresses, and at the same time, he called one of his acquaintances surgeons working in the private clinic "Avicena", with whom he agreed that he would receive me tomorrow December 28th.

Having returned home, I again sat down at the computer. I had previously studied and accumulated articles on cancer treatment methods, and here, having returned to them, I decided: to categorically change my diet. Although malnutrition is not the primary detrimental cause in this case, I did not smoke or consume alcohol, so I did not have the primary reasons for the development of malignant neoplasms. But malnutrition is also an important background for the onset of cancer. Especially today, when manufacturers use various methods to make a profit, significantly worsening the quality of food. The first thing to do was to refuse the consumption of animal protein: meat, and any meat products including poultry, milk and any dairy products. And I immediately decided in the future not to use these products. It was not so difficult for me. But I also read that cancer can develop as a response of the body to the development of fungi, and bacterial infections or parasites. And for them, the nutrient medium is sugar. And I consumed a lot of sugar and sugar-containing products. I really loved sweets. When I drank tea, I put three teaspoons of sugar in a 200 gram glass. And I had to give up all products containing sugar. At first it seemed to me that it would be very difficult to do, but having embarked on this path, I managed to cope with it. And today I am not even “drawn to” sweets. And this process reminds me of the one that happened to me in 1984-1985, when I refused to drink alcohol. In addition, over time, I had to abandon the use of yeast bread. And today, when someone hears from me that I don’t consume all these products, she asks: “what do you eat?” I answer that everything except the ones listed above, and at the same time, today I don’t feel a real limitation for myself in this. The only thing that bothers me is that there is no guarantee of the quality and safety of those products that are sold. Today, for the sake of profit, sellers are doing everything, including the sale of products that are unhealthy. Therefore, in Novosibirsk, but I think throughout Russia, a network of environmentally friendly products is in great demand, even if it is twice as expensive, if only it is reliably harmless to humans and fresh.

So, starting from the first days of January 2013, I switched to a new diet. But before the New Year, I managed to pass all the tests, make tomography and ready-made results to bring to the urologist. He scheduled a TUR operation for me to take a biopsy on January 6, 2013.

Before the operation, I continued to study all the directions that exist in the world for the treatment of cancer patients. He turned to materials previously collected at home.It became clear that the main cause of oncology - weakening of the human immune system. And all effective methods are based primarily on the normalization of the immune system. And here it was necessary to immediately separate all the methods offered by various clinics, healers, etc. to strengthen and weaken the immune system. But the methods that treat cancer only through strengthening the immune system require a large margin of time, and it is not always enough for cancer patients. Therefore, for each specific patient, a combination of these methods with methods of specific local effects on the tumor is needed. Therefore, the fact that I went on a TUR operation then, and now I evaluate it as an absolutely right step. This gave me a margin of time for experiments to boost my immune system.

It turned out that the usual morning exercises for at least 30-40 minutes a day, contrast douche - greatly increase the level of protective functions of the human body. Therefore, even before the operation, I developed a set of exercises focused on my illness, and began to do exercises and contrast douche every morning. At the same time, I planned to gradually increase the intensity and number of exercises. And I succeeded. Since that time, I start almost every morning with intense charging and contrast douche.

But back to those days. Even before the operation, in my computer I found a selection of articles about the method used by Viktor Alexandrovich Borisov. I studied these articles again, recalling that several years ago I even talked on the phone with him. Firstly, Borisov V.A. professional doctor, candidate of medical sciences. Secondly, he works in a rehabilitation center, where he is involved in the rehabilitation of cancer patients. Thirdly, he personally has been dealing with this problem for more than 20 years. Well and most importantly, his method, not only does not inhibit the human immune system, but on the contrary increases its level and this is done in a targeted way and when the tumor itself is affected. Therefore, I realized that I need to talk to V.A. again Borisov and his specialists, in order to finally understand: to take this method to me in my arsenal of fighting cancer or not.

Waiting on January 6, I went to the Avicena clinic early in the morning, where I was scheduled for a TUR operation, with tissue sampling for analysis. Already on the same day, under general anesthesia, I had an operation and the part of the tumor that had grown inside the bladder was removed - it amounted to more than 12 cubic centimeters. The remaining part of the tumor of at least 10 cubic centimeters - has grown into muscle tissue. On January 8, I was discharged from the clinic with a catheter, but on January 10 I arrived with a catheter and they removed it. During this time, I again contacted Borisov V.A. and agreed to undergo a course of therapeutic therapy in his clinic. At the same time, in the Avicenna clinic, I was referred for consultation with oncologist Talovsky Yan Raymondovich. He persuaded me to undergo 6 courses of chemotherapy. But since, after I understood the most important thing in the treatment of oncology: everything should be aimed at strengthening the immune system, and any chemistry or radio therapy can cure it, then in no case can it be used for treatment. And the method that A.V. uses Borisov (biochronotherapy), not only does it act locally on the tumor, it also strengthens the immune system. Therefore, I had no doubt whatsoever in choosing the method that should be used to treat my disease.

I must say that when my friends found out that I had such a sore, they began to offer various options, for which I am grateful to them. All that I was offered was really working drugs, but they should work in a common system: nutrition, drinking, tempering, psychological stimulation. These are drugs such as Vetom -1.1, Alpha-fetoprotein, Transfer factor, etc., they can only restrain the development of oncology, but fundamentally they do not solve the problem.

Already on January 20, 2013, I flew to Moscow and from January 20 to 26 I took a course of 6 sessions of biochronotherapy. The essence of this method is as follows: a sensitizer consisting of drugs containing chlorophyll is administered rectally and intramuscularly or intravenously into the patient’s body. Oncological cells are intensively "fed up" with chlorophyll. After that, certain places in the patient’s body are affected by infrared radiation of a certain frequency and intensity, depending on the respiratory rate and the pulse undergoing the procedure. Under the influence of radiation, chlorophyll decomposes and destroys oncological cells from the inside. At the same time, control is carried out by immunological parameters of blood.

I forgot to say that by my flight to Moscow I received an opinion on my biopsy and the primary materials themselves with tissue sections on the “glass”. It turned out that I had the third stage without metastases and without inflamed lymph nodes, but “infiltrative urothelial carcinoma of high grade G3 with germination into the muscle layer within the taken organ wall” (bladder cancer p N3 N0 M0 G3).

The results show the operation: radical cystectomy (completely cut the bladder, prostate and testes and form an artificial bladder and walk with the urine collector) With these materials, I flew to Moscow to them. Herzen turned to the main pathologist of the Ministry of Health and Social Development of Russia, Academician of the Russian Academy of Sciences Georgy Avraamovich Frank. He personally checked the biological materials I had brought under a microscope and confirmed the diagnosis made by Novosibirsk doctors.

When I received the results of my biopsy in Novosibirsk before traveling to Moscow, I sent copies of my tests to clinics in Israel and Germany. And having returned from Moscow, by e-mail I received an invitation from them for treatment. But the doctors from these clinics offered me only one option: completely cut out the bladder, prostate and testes and form an artificial bladder and walk with the urine collector, that is, the same scenario as the Novosibirsk doctors. At the same time, chemotherapy courses were also required. After talking with doctors and oncologists, I realized that in this scenario, the probability of living 5 years is 50%. Therefore, I immediately deleted this scenario from my promising arsenal. At the same time, the doctors told me that if I did not follow this scenario, then I would not live for more than two years. And nevertheless, relying on the work and materials studied during this time, I finally made a decision: neither any chemotherapy, nor any organ removal.

While in Moscow, I also managed to negotiate through my friends with the chief physician of the Civil Aviation Hospital with Zabrodina Natalya Borisovna that they were ready for my next visit to Moscow (I planned to come to a new course to Borisov VA in his rehabilitation center in a month ) conduct another TUR operation and try to maximally cut out the remnants of the tumor that has grown into muscle tissue. Although having looked at all my analyzes, she honestly said that I have very few chances for a positive outcome, but there are some.

Three weeks later, having arrived in Moscow, I took a course at the Center at V.A. for 6 days. Borisov and then went to the hospital to N. Zabrodina I was appointed a doctor Lukin Andrey Viktorovich. From February 25, 2013 to March 1, 2013, I was treated at the Central Clinical Hospital of Civil Aviation. During this time, I had a TUR operation. The surgeon A. Lukin operated. Since the catheter was blocked several times after the operation and they were changed 4 times during the night, an infection was apparently brought in, which I later had to fight in Novosibirsk later.I want to say that the operation was carried out at a high professional level, since I had to actually cut through the muscle tissue of the bladder. And on March 1 I flew to Novosibirsk, and on March 3 at 168 medical units I removed the catheters.

Analyzing from today's perspective, I think that this second TUR operation could not have been carried out. But again, this is my assumption based purely on intuition. There is no evidence that if I had not given the treatment then and I had everything — well, I don’t have it.

After that, I also conducted three courses of biochronotherapy at the Center under the guidance of V. Borisov. : in March, April and October 2013. At the same time, in October 2013, I did it this way - for a guarantee. According to my current knowledge on this subject, in October the course could not have been taken.

Further, from mid-March to May, I was involved in removing the infection from the genitourinary system and restoring normal urine retention. Surgery is not the most optimal treatment. They need to be resorted to only when the amount of time to restore the immune system is simply not enough.

Yes, I forgot to write about the fact that since March I switched to increased consumption of unboiled water. Today I drink from 2.5 to 3 x liters of water per day. This is a very important point not only for treatment but also for the prevention of oncology. Be sure to wake up and immediately drink 300-330 grams of water. This allows you to immediately wake up and be awake. For the bladder, this does not allow to increase the concentration of harmful substances in the excreted urine and does not allow it to stagnate. In addition, it allows the kidneys to perform their function.

Subsequent TUR biopsies in June 2013 and June 2014 showed no development of bladder cancer.

It must be said that in July and in October 2014 I experienced another active method of treatment and prevention of cancer. The so-called cancer vaccine developed by Britov Vasily Alexandrovich. He believed that in the initial stages 1-2 this method is an effective treatment method. But its main feature is that those who received this vaccination did not get sick in the subsequent disease. But recently, after several cases, I came to the conclusion that with the combination of the path I described and with the use of the vaccine, Britova V.A. cancer of the 4th stage is also taken.

I must say that as a miner walking through a minefield I was looking for a cure for myself, studying various scientific and popular science articles, rejecting and choosing only those that led me to salvation. Each of my steps and recommendations is based on certain knowledge and experience of those doctors and scientists who themselves have gone the way of healing or have cured more than one hundred people. Therefore, in the next chapter, I will simply give specific recommendations without reference to work and explanation of what needs to be done and what cannot be done to cure cancer. And already those who want to understand why it is necessary to do this will write the last chapter of this article.

Treatment of a tumor of the bladder with folk remedies

Treatment of bladder cancer with folk remedies is allowed only as an addition to traditional methods of therapy. A cancerous tumor requires complex treatment.

The composition of such treatment also includes folk remedies, but only on condition that there are basic procedures, that is, treatment is carried out by the traditional method. Herbal treatment has a restorative, antitumor and immunomodulating effect.

But mindlessly using herbs to treat the tumor is also impossible, since they must be combined with each other and the patient should not have contraindications to their use.

Disastrous ignorance

Bladder cancer in the first stage is curable in 95% of cases, in the second - in 75%. But with widespread cancer (with metastases), the situation is extremely sad.Only 6% of these patients live longer than 5 years. Therefore, early diagnosis is extremely important!

Unfortunately, today 34% of patients with bladder cancer turn to urologists more than six months after the first symptoms. And in one of 10 patients, by the time the diagnosis is made, an advanced form of cancer is recorded.

But to catch a dangerous disease at an early stage is not difficult. To do this, it is enough to take a general urine test once a year (to detect microhematuria - an increased number of red blood cells) and if dangerous symptoms appear, immediately go to the urologist! The reason for not postponing a visit to a specialist may be: pain in the abdomen and lower back and frequent urination, and especially the appearance of an impurity of blood in the urine. And, although such symptoms are not always associated with oncology (these may be signs of cystitis, urolithiasis, or, for example, an overactive bladder), it is better to be vigilant. In addition, all of the above pathologies can also not be ignored by a doctor, because they can provoke cancer.

Chemical and immune therapy

Chemotherapy is the use of drugs to kill cancer cells. Anticancer drugs are usually released throughout the body (systemic therapy) to destroy the inaccessible cancer cells that have spread and circulate in the body. Chemotherapy is based on the principle that cancer cells are more severely affected than normal cells because they divide rapidly. Chemotherapy drugs can be injected into a vein, muscle, or skin, or they can be taken by mouth.

When chemotherapy is used before surgery, it is known as primary chemotherapy. Its goal is usually to reduce the size of the tumor. In this form of treatment, chemotherapy is performed after surgery to destroy any remaining cancer cells and helps prevent cancer recurrence. Chemotherapy can also be used in combination with radiation therapy. Side effects of chemotherapy vary, but may include susceptibility to infections, fatigue, poor appetite, weight loss, nausea, diarrhea, and hair loss. A decrease in fertility can be a long-term side effect in some patients who are undergoing chemotherapy.

Immunotherapy, also called biological therapy, is the use of treatment methods that promote or support the body's immune system response to cancer. Side effects of this immunotherapy vary, but include flu symptoms, weakness, loss of appetite, and skin rash.

Treatment of a bladder tumor is a complex and time-consuming process that ends with a long recovery of the patient. To do this, you need to make a lot of effort, endure painful procedures and go through various methods of therapy. Folk remedies in this case do not occupy the last place, especially in the early stages or during the recovery period after surgery.

Oncological diseases in modern society firmly hold a leading position. Any malignant tumor is a threat to human life. Experience of others

After I was cured myself, I began to send out my article to everyone who wanted to be cured. And a few people went this way. The latter case is especially interesting. I was approached by a friend of mine with whom we went together to the "barricades". He turned to me in March 2015. His wife (84 years old) has stage 4 breast cancer. At the first and second stages of the disease, the following methods are usually recommended:

Transurethral resection (TUR). it sparing The operation is used to remove small malignant tumors that have not spread beyond the cells of the inner membrane. It is performed in a closed manner, through the urethra.

Partial cystectomy - surgical excision of the neoplasm and a small fragment of adjacent tissues. This method is used if the area of ​​the wall affected by the tumor can be easily removed without affecting the function of urination and retention of urine.

Biological therapy (immunotherapy). Immunotherapy is often carried out in combination with surgery in the first and second stages. BCG is introduced, sometimes interferon Alfa-2B into the body through the urethra.

At later stages of the disease, the doctor may recommend the following methods:

Radical cystectomy - removal of the entire organ with nearby lymph nodes. Treatment of bladder cancer in men with this method, as a rule, involves the simultaneous removal of the prostate gland with seminal vesicles. In women with radical cystectomy, the uterus with ovaries and part of the vagina are usually removed.

Immediately after surgery, the surgeon creates new mechanisms for the removal of urine. It could be:

  • a reconstructed organ that connects to the urethra or urostoma,
  • ureterostomy (an artificial hole for the removal of urine into the urinal).

Radiotherapy. Radiation therapy for bladder cancer can be chosen as an auxiliary method in conjunction with surgery or chemotherapy. In the treatment of the bladder, both remote radiation therapy (RT) and contact radiation (brachytherapy) are prescribed. Sometimes this method is used instead of surgery or chemotherapy.To increase the effect of RT, special agents (sensitizers) can be prescribed that increase the sensitivity of tumor cells to radiation.

Chemotherapy. It can be either systemic or intravesical, i.e. the medicine is injected directly into the organ itself. Intravesical chemotherapy is used for bladder cancer to provide cytostatics directly to the tumor site. Chemotherapy is often used with an integrated approach and is prescribed in the preoperative period to reduce the tumor or after surgery to destroy the remaining cancer cells.

An aspiration biopsy of the enlarged lymph nodes as an independent procedure under the control of computed tomography can be performed to confirm the existence of inoperable metastases in the lymph nodes.

It is believed that pelvic lymphangiography has no advantages over computed tomography and determines metastases in 50-60% of cases, while being an invasive method. CT can be used in conjunction with a fine-needle aspiration biopsy of the pelvic lymph nodes to determine, but not exclude, the presence of metastases in them.

The method of nuclear magnetic resonance (NMR) scanning and its modern technical methods make it possible to clarify the stage of the disease and assess the condition of the bladder wall, paravesical space, surrounding organs and systems 11, 47. According to Kim B. et al. , computed tomography and nuclear magnetic resonance are suitable for the detection of bladder tumors, but errors in establishing the stage of the disease occur in 55% of cases.

It is assumed that photodynamic research can become the basis for the traditional diagnosis of bladder cancer 29, 79. This method for diagnosing intraepithelial lesions is important for determining the presence or absence of precancerous or cancerous lesions of the mucosa and does not give false-positive results.

The listed set of diagnostic methods is not necessary for all patients with bladder cancer. Research should be carried out individually and deepened as necessary to clarify the diagnosis and optimize treatment.

So, in the literature we did not find information about the need for a thorough determination of the location of the tumor before radiation therapy for its targeted exposure.

In addition, many patients before plastic replacement of the bladder with a segment of the intestine need careful urodynamic studies to ensure an adequate mechanism for retaining urine after surgery. In certain cases, a special examination of the upper urinary tract is necessary.

  • Bladder Cancer Treatments

Conservative treatment: indications and “types”. There are few reports of conservative treatment for bladder cancer in the literature. Conservative treatment is indicated for non-epithelial tumors of the bladder (neurofibroma) until there is no pronounced symptomatology. In primary lymphoma, rhabdomyosarcoma, radiation and chemotherapy can be performed.

Photodynamic treatment was reported (after intravesical administration of photofrin or photosan-3, irradiation after 48 hours with a light dose of 15-30 / cm2 wavelength of 630 mm) as an alternative cystectomy treatment method for refractory (not responding to BCG and chemotherapy) bladder cancer (multifocal surface). At the same time, out of 23 patients, only four required cystectomy surgery.

Various therapeutic modifications of the conservative treatment of invasive bladder cancer have been described, and it is believed that bladder retention is possible only in some cases.The protocols on the role of chemotherapy and combination chemotherapy are considered to be incomplete, therefore radical cystectomy is the gold standard in the treatment of invasive bladder cancer.

Transurethral resection (TUR) in the treatment of bladder cancer. TUR is possible in patients with small, localized tumors with superficial muscle invasion or in patients with contraindications for cystectomy 12, 47, 84. TUR is performed in patients with stage I - II bladder cancer (T1 and T2), and in later stages (T3 ) with a palliative purpose.

Most patients with superficial bladder cancer can be adequately treated with TUR or fulguration. The results of a five-year observation showed that 70% of these patients are cured, while approximately 10-15% of these patients need more intensive therapy. Cases of subsequent muscle invasion are rare in patients with initially limited mucosal tumors, and can occur in 46% of patients with tumors involving the submucosal layer. Accordingly, patients with T1 should be treated as patients with potentially aggressive tumors, especially with a decrease in the degree of cell differentiation (G2-3).

An increase in the depth of tumor invasion in patients with primary T1 bladder cancer (Holmang S., 97) is associated with a worsening of the long-term prognosis. Therefore, with deep invasion (pT1b stage), the effectiveness of only TUR surgery is very low.

Patients with superficial bladder cancer after TUR should be monitored dynamically with cystoscopy every three months for two years and every six months for the next two years. They also need to carry out excretory urography 29, 47 annually. The volume of endoscopic intervention for TUR is determined by the form of tumor growth and is carried out by a “stepwise” biopsy. Patients with superficial bladder cancer are divided into risk groups, on the basis of which criteria for the need for intravesical treatment are formed. Tumor relapse in patients from a low or intermediate risk group can be significantly reduced after a single intravesical monochemotherapy.

Bladder TUR can be considered as a diagnostic procedure and at the same time as the initial stage of treatment for bladder cancer.

An endovascular biopsy from the wall of the bladder from the TUR zone is of great diagnostic and prognostic value. It allows confirming the radical nature of the performed endosurgical intervention 4, 10. If a tumor invasion is detected, radical surgical treatment can be undertaken or, in the case of advanced age and extremely severe concomitant diseases, radiation therapy. A multifocal biopsy makes it possible to detect cells capable of resuming growth without additional therapy, including radiation, chemotherapeutic and immune methods of exposure. Such an integrated approach provides organ-preserving treatment of a number of bladder tumors. Tsukamoto T. et al. (1994) consider that conservative therapy is acceptable for T1b tumors, as more than half of the patients were successfully treated with TUR. Taking into account the true local recurrence due to incomplete resection of the area of ​​primary damage, the authors indicate the need for re-resection of re-TUR.

Some authors define TUR with adjuvant chemotherapy according to the MVAC scheme as the main and radical treatment of patients with primary and recurrent bladder tumors in the stage pT2 G1-2 and the primary tumor pT3aG1. In the later stages, TUR is a palliative operation. The authors consider this treatment possible with large tumors, obstruction of the ureteral orifice, impaired urodynamics of the upper urinary tract and the presence of chronic pyelonephritis according to the experience of TUR, with intravesical BCG therapy, adjuvant chemotherapy and radiation therapy. Patients with superficial high-risk bladder cancer (documented multiple recurrent transitional cell carcinoma) require aggressive local therapy. Despite its conduct (TUR + BCG), patients are at high risk of disease progression and death from bladder cancer, the development of tumors of the upper urinary tract.Nevertheless, based on a 15-year follow-up, it can be argued that the survival rate in this case is 62%, which is comparable to cystectomy treatment.

Thus, TUR in bladder cancer can be used as an independent treatment for superficial forms of cancer and in combination with immuno-, chemo- and radiation therapy. TUR is a procedure that clarifies the diagnosis, and therefore may require a change in treatment tactics. In addition, TUR can be used as a palliative treatment manipulation.

Immunotherapy, chemotherapy, radiation therapy for bladder cancer. Patients with localized superficial bladder cancer are identified by risk groups, according to which chemotherapy or immunomodulating treatment is additionally prescribed after TUR.

BCG therapy after organ-preserving operations is considered as an effective prevention of relapse of bladder cancer and has the actual therapeutic effect against superficial forms of the tumor. The relapse rate was lower in the period up to 1-2 years in patients with BCG therapy - 27% compared with 80% of patients who were not given additional therapy.

At a five-year follow-up period, the number of patients who did not have a relapse of the bladder tumor after TUR with immunotherapy increased 3.4 times compared with patients who did not receive additional therapy.

The use of local chemotherapy and immunotherapy in combination with antibacterial drugs can delay the occurrence of relapse of bladder cancer 13, 14. A decrease in relapse rate with the least number of complications was confirmed in 85 T1 N0 M0 patients with a moderate degree of cancer cell differentiation after local chemotherapy.

At T0-1 with a high and moderate form of cancer cell differentiation, BCG therapy is effective. Of the complications, a chemical burn was observed in three patients, one had tuberculous orchoepididymitis after BCG therapy 13, 14.

There is evidence of the effective use of BCG vaccine in the postoperative period in patients with superficial bladder cancer.

TUR followed by (adjuvant) intravesical chemo- or immunotherapy, which reduce the likelihood of relapse, is called the “gold standard” in the treatment of superficial forms of bladder cancer. The toxic manifestations of local chemotherapy in the form of cystitis increase with increasing dose of the administered pharmaceutical drug. It is important that intravesical chemotherapy reduces the rate of recurrence. It is not proven that it causes the progression of tumor growth.

The mechanism of immunotherapy consists in a specific and non-specific reaction to inflammation. Complications may be associated with catheterization trauma. BCG has advantages over chemotherapy, as it reduces the rate of progression, which is explained by a deeper course of the inflammatory reaction to the introduction of an immunotherapy.

With tumors at high risk of recurrence of pT1G3 with or without carcinoma in situ, BCG therapy is possible. In the early stages of the tumor (T1 - T2), TUR in combination with remote irradiation provided a five-year survival rate of 54.5%, this indicator in patients undergoing bladder removal with subsequent irradiation was 28.5%. At the same time, with a more widespread process, better results were obtained by combining radical surgery with radiation.

In combination with operative treatment, interstitial radiation therapy is also used. For tumors with a diameter of not more than 5 cm, performing TUR with a course of external irradiation at a dose of 30 Gy and additional interstitial implantation made it possible to achieve that within five years there were no local relapses in 84% of 40 patients.

A randomized study of 443 patients with superficial transitional bladder carcinoma of the bladder, whose purpose was to determine the time of onset of the first relapse and the rate of relapse within a year, showed that adjuvant chemotherapy (intravesical doxirubicin and / or etoglucid) was superior only to TUR. However, the progression and survival rates did not differ for both treatment methods. Thus, organ-preserving treatment of TUR bladder cancer with radiation or radiochemotherapy is possible and effective, as Dunst, J.(1994) proved that survival is comparable to cystectomy.

Surgery. Surgical treatment of invasive forms of bladder cancer remains one of the challenges of oncourology 13, 14. This method is the main, radical method of treatment, and the choice of the optimal volume of surgery is based on information about the localization and prevalence of the tumor process, the morphological structure of the tumor and the patient's somatic state.

There are two approaches to the surgical treatment of bladder cancer: the first is bladder preservation, and the second is bladder reconstruction. The purpose of the first approach is to cure cancer, ensuring normal bladder function. Organ-saving treatment tactics are most suitable for patients with minimally invasive transitional cell carcinoma with a high degree of differentiation. It should be borne in mind that subsequent cystectomy for a residual tumor after irradiation and chemotherapy is associated with a greater risk. The choice is difficult, since so far there are no comparative data on both organ-preserving and reconstructive surgical treatment protocols.

In the second approach, the initial treatment consists of radical cystectomy followed by urine diversion or organ reconstruction and additional chemotherapy in patients with a histologically confirmed risk of tumor recurrence. The benefits of this treatment approach, according to Catalona W. J. (1998), are related to the desire to avoid the need for chemotherapy in patients who do not need it. However, chemotherapy is poorly tolerated by patients in the postoperative period.

Organ-preserving cystoplasty. According to V. Zhuravlev, (1998), indications for bladder resection to the entire thickness within healthy tissues are tumors in the T2 3N0 M0 stage, located far from the neck and mouth of the ureters. Wedge-shaped resection can take place with bulk tumors of the side or front walls. Resection with ureterovesikoanastomosis according to Fedorov is possible with a lesion of one of the mouths of the ureter without signs of hydronephrosis, and with hydronephrotic transformation, an operation according to Politano - Leadbetter is indicated. Hemicystectomy with expanded lymphadenectomy can be performed with multiple tumors of one of the side walls with or without germination at the mouth of the ureter 13, 14.

Partial cystectomy is performed in 10-15% of patients with solitary invasive bladder cancer, if TUR is not possible. At the same time, a biopsy of the mucosa is taken to make sure that there is no severe atypia and carcinoma in situ, the resection is performed 2 cm around the tumor, followed by low-dose irradiation to prevent the implantation of tumor cells into the surgical wound.

Sergienko N.F. (1998) with moderate and highly differentiated bladder cancer of stage T2 prefers bladder resection, since after these operations relapses are diagnosed only in 43% of patients, and after TUR in 65%. Kolesnikov G.P. (1998) considers it appropriate to perform an extended resection if possible with subsequent combined and complex treatment, since the survival rate of patients after bladder resection is higher than after cystectomy, despite the high risk of relapse and the need for repeated treatment.

With non-epithelial tumors of the bladder, such as pheochromocytoma, partial cystectomy is possible, and it is necessary to monitor the state of the lymph nodes. With leiomyosarcoma and small-volume tumors, enucleation or partial cystectomy is indicated, with large-volume tumors - total cystectomy. In rhabdomyosarcoma, surgical treatment is carried out due to the low sensitivity to chemo- and radiation therapy.

Cases of lethal outcomes within 14 months after partial cystectomy (without additional treatment methods) for small cell bladder carcinoma (known as oat cell carcinoma), without obvious signs of extravesical spread, are described.

Radical cystectomy. Radical cystectomy is an effective method of treating a primary bladder tumor, as it is accompanied by the lowest relapse rate (10-20%) compared with (50-70%) radiation, chemotherapy, or a combination of both. Radical cystectomy should not be considered the “last resort” after unsuccessful attempts to control the tumor using organ-preserving operations, but should be performed according to clearly established indications in the early stages of bladder cancer. With sarcoma, angiosarcoma, bladder cancer, radical cystectomy is performed in the absence of metastases.

The number of patients requiring radical treatment is constantly increasing. Radical surgery, according to Witjes J. A. (1998), should be performed in relapses three to six months after intravesical therapy if tumor aneuploidity is proved or expression of the p53 peptide is impaired. The treatment of infiltrative local carcinoma, invasive bladder cancer is radical cystectomy 84, 99. A cystectomy with bilateral ureterocutaneostomy is indicated for total bladder papillomatosis with germination in the neck and triangle 13, 14. Detection of transient cell carcinoma of the bladder in the prostate gland partial cystectomy or endoscopic treatment and intravesical chemotherapy are impossible, and is also a contraindication for the creation of internal urinary tract uarov, anastomosed to the urethra after radical cystectomy. Assessing the long-term results of the treatment of stage T1b and T2 bladder cancer, Tsukamoto-T., Et al (1994) believe that radical cystectomy should be performed as an eight-stage treatment of second stage tumors (T2), arguing that out of 8 patients who underwent TUR treatment, only one did not have a relapse. The rest died from tumor progression, whereas after cystectomy a life expectancy of 5 and 10 years was achieved in 64 ± 14% of the operated.

Some authors consider it appropriate to carry out radical cystectomy with superficial bladder cancer 19, 20, 53. Indications were the presence of multifocal primary and recurrent G2-3 cancer, invasion of the bladder neck and urethra, and the ineffectiveness of the previous complex treatment. The experience of 37 radical cystectomies, including total removal of the prostate gland, allowed the authors to consider a two-stage treatment with the separation of cystectomy and intestinal plasty as preferred, as the simultaneous execution of these operations leads to high early postoperative mortality (4 out of 11 patients) 19, 20. Kogan M. I et al. (1998) give preference to orthotopic sigmocystoplasty (as compared with over-bladder abduction) with mandatory detubularization of the intestinal segment for adequate function of the formed intestinal reservoir and reducing the risk of urethral tumor recurrence 19, 20. A critical analysis of various prognostic factors affecting patient survival (764 men and 262 women, observations up to 24 years, on average 4 years) with bladder carcinoma and muscle invasion showed that simultaneous cystectomy with retention leads guarantees m minimality trauma, good local control and high-performance life. The defeat of the regional lymph nodes is not a contraindication 63, 64.

The development of urinary retention leads and their surgical preservation of the innervation of surgical aids are due to the benefits of a bladder reconstruction operation.Mortality after radical cystectomy decreased from 20 to 2–0.5% (with chemotherapy 2–6%).

Methods of derivation of urine. Managadze L.G. et al. (1994) proposed a detailed classification of supravesical urine derivation, which takes into account the technique of operations, the anatomical and functional features of surgical approaches and greatly facilitates the understanding of historically established and existing methods of urine diversion. According to Catalona WJ (1998), the following methods of urine diversion are currently used: 1) - the ileal conductor, 2) - permanent skin leads, 3) - holding orthotopic reservoirs to the urethra with the ileum-intestinal or iliac-cecum reservoirs, which are formed only in men, 4) - increased rectal reservoirs, and in women, too. Analysis of the results of urine diversion in patients with a removed bladder in the stage of cancer T3 and T4a (ureterocutaneostomy, into the sigmoid and rectum, into the isolated intestinal segment with a colostomy) by the activity of pyelonephritis, the degree of renal failure, and the severity of the postoperative course allowed Shirokorad V.I. et al. (1998) consider ureteroileoanastomosis, isolated ileocecal angle with a “dry” stoma and ureterosigmorectoanastomosis as the method of choice. The accumulated experience allows us to consider the two types of surgical interventions as justified: transplantation of the ureters into an isolated segment of the intestine with its output to the skin and the formation of an artificial intestinal bladder with a preserved act of urination, which seems to be an ideal solution. When creating a reservoir with urine diverted to the anterior abdominal wall, a “dry” stoma can be formed due to mechanisms that inhibit spontaneous outflow of urine (Bauginia damper, narrowing of the intestinal lumen, intussusception, for example, isoperistaltic ileo-ileal). The localization and prevalence of the tumor process are decisive in the choice of the method of derivation of urine in patients with bladder cancer. Only in the absence of damage to the bladder neck and invasion of the tumor into the prostate gland, it is advisable to create an artificial bladder, since for the good formation of the intestinal-urethral anastomosis and preservation of the function of the external sphincter, it is necessary to leave part of the prostate. In patients with damage or failure of the sphincter of the bladder, ureterocecoileocutaneostomy should be considered the operation of choice.

Urine diversion to an isolated ileal segment of the ileum (ileal conduit) is the simplest surgical procedure, but requires specific preparation 41, 111 from the patient. This technique is indicated for severe patients with a high risk of postoperative complications 38, 111.

The experience of such urinary diversion over 17 years in 185 patients in connection with bladder cancer showed a survival rate of 84, 72, 67, 62 and 54%, respectively, for 1, 3, 5, 10 and 15 years. Early complications in 38% of patients are associated with delayed healing due to local infection and bowel obstruction. Other authors noted inflammatory complications, hernia, anastomosis stenosis, strictures, stones, pyelonephritis. Among late complications, 51% of patients noted near-stomal dermatitis, gradually developing disorders of the upper urinary tract and kidneys 47, 90. Hydronerosis was observed in 13% of patients with normal preoperative urograms. Ileoureteral reflux was diagnosed in 50% of patients with non-obstructive methods of assignment and in 70% of patients with obstructive methods.

Complications of performing jejunum leads are due to the loss of sodium and chloride. Transverse colonic intestinal leads are used in patients with postradiation enteritis. The colon (colon) is more convenient for non-refluxing uretero-intestinal anastomoses, but strictures often occur. Olivo G.considers ileal conductors are more successful from the metabolic point of view, than ileocephalic vesicles. The risk of developing cancer with sigmoid cystoplasty is lower than with ileum (Buson H.) and even lower after colocystoplasty 44, 82.

The cause of bilateral hydroureteronephrosis after urine diversion into the ileum is stenosis of the ureteral orifice, stoma stenosis and poor compliance of the segment. A standard estimate of the size of the stoma, residual urine in the drain and the lupogram does not always reveal the cause of the expansion of the ureters. A study of the pressure above and below the fascia when filled with a contrast catheter with a triple lumen revealed segment dysfunction in five of six patients with hydroureteronephrosis. The study of peristalsis under X-ray control allows you to identify pressure rises that are the cause of reflux, and areas of atony, which is why complete emptying is not achieved and the risk of stone formation is created. In such cases, it is recommended that you remove the old loop quickly and create a new segment 77, 90.

In recent years, the role of intestinal-iliac urinary diversion in patients with bladder cancer has been reduced due to unfavorable long-term results (stoma dysfunction and expansion of the upper urinary tract) and the emergence of new treatment methods with self-catheterization and bladder replacement that improve the quality of life. The use of an isolated segment of the sigmoid colon for ureterosigmocutaneostomy has certain advantages, since the sigmoid colon is topically and genetically close to the bladder, it secretes less mucus and urine reabsorption is less pronounced.

Ureterosigmoanastomosis has a number of negative aspects: reflux, obstruction and stones of the ureters, electrolyte imbalance, hyperchloremic acidosis. Fecal and urinary streams pose a risk of mucosal adenocarcinoma, therefore, separation of flows is advisable 7, 47, 51, 75, 98, 106.

There is a technique for the simultaneous removal of urine and feces through a common abdominal opening using a loop of the colon 15 cm long distal to the opening with internal flow separation. In contrast to a wet colostomy accompanied by diarrhea, the manual noted minimal complications: there was no pyelonephritis, problems with electrolytes, hydronephrosis (observation of 11 patients from 0.5 to 80 months).

Correlation of the occurrence of cell metaplasia with urinary infection has not been confirmed by other authors. Bilateral ureterosigmoid anastomosis is one of the simplest and most reliable methods of urine derivation for cystectomy 2, 42. It is indicated for patients with a limited life expectancy, whose somatic state does not allow for a more complex and prolonged surgical intervention. Ureterosigmoanastomosis (stomy) is indicated for patients who want to have a retaining form of internal urinary diversion, despite electrolyte imbalance and urinary infection. For seven years, the authors performed 19 operations for tumors of the bladder: transitional cancer in 17 and squamous cancer in two patients. Postoperative electrolyte fluctuations (K and Na serum) were within normal limits. Hyperchloremic acidosis was corrected with 10% potassium and sodium citrate. Pyelonephritis was diagnosed in 21% of patients. Excretory urograms were detected in 4 of 19 patients with mild / moderate hydronephrosis two months after surgery, which resolved within a year. Normal life without external urological devices was carried out by 10 patients, 7 operated on died from cancer recurrence, 2 with a follow-up period of up to six years - from pneumonia.

Studies of the long-term results of ureterosigmoanastomosis surgery showed that after seven and a half years, all patients felt comfortable. Electrolyte imbalance was normalized by routine administration of sodium bicarbonate.Gut carcinomas were not observed, which allowed Wynant H. P. (1991) to consider the technique of ureterosigmoanastomosis undeservedly unappreciated.

Ureterosigmoanastomosis is possible if the violation of the urodynamics of the upper urinary tract does not exceed the second stage. With a more pronounced decrease in the tone of the urinary tract, it is advisable to perform transureteropyelo-ureterosigmoanastomosis, which reduces the infection of one of the kidneys with intestinal flora 2, 3. In patients with loss of the ureters due to previous operations or radiotherapy of the pelvis transverse intestinal (urine-pyelotransverse conduit) diversion a surgical solution more convenient for patients than bilateral nephrostomy.

Retention diversions of urine do not require an external collecting device. This technique uses intestinal segments to create a reservoir released by catheterization through the retaining abdominal stoma, or a reservoir that anastomoses with the urethra to form an orthotopic new bladder.     Surgery is the main treatment for bladder cancer.With a small tumor that does not affect the muscle layer of the wall, you can limit yourself to removing only a portion of the bladder wall (resection). This type of operation takes place without incisions using a special instrument inserted through the urethra.

    With a significant spread of the oncological process, only a radical removal of the bladder will help cope with the tumor - cystectomy. In addition, excision of the prostate, uterus with appendages, and the nearest lymph nodes is often used.

    The second step is to create a new reserve for storing and diverting urine. Most often, a section of the intestine is used for this purpose. While maintaining the integrity of the urethra, the patient will be able to subsequently control urination. Otherwise, the outflow of urine is carried out through an opening in the front wall of the abdomen in a special receiver.

    In the case of an extensive spread of the tumor, including the urethra, the outflow of urine is carried out through a special hole above the pubis - epicystostoma. Radical intervention in this situation is not carried out. When a tumor breaks up, cystoscopy is used to stop bleeding from its vessels, supplemented by cauterization (coagulation) of the problem area.

    Medication

    The appointment of anticancer drugs aims to reduce the number of malignant cells and the prevalence of the process. Chemotherapy is used both before and after surgery to prevent relapse. The following drugs are used:

    • Pharmorubicin
    • Vinblastine
    • Cyclophosphamide,
    • Mitomycin.

    Chemotherapy drugs are often administered intravenously, but in some cases they are delivered directly into the bladder using special instruments through the urethra. The drug is then released slowly, exerting an antitumor effect.

    To eliminate bleeding from a malignant tumor, hemostatic drugs are used: Dicinon, Ethamsylate, Aminocaproic acid. With a significant loss of blood, transfusion of donor components is used - erythromass, plasma. With severe pain, pain medications are prescribed:

    Immunomodulators decrease the rate of tumor progression. For this purpose, BCG vaccine is injected into the bladder cavity. The latter is also used to prevent tuberculosis. In response to its introduction, local immunity is formed. Immune cells inhibit the growth of malignant neoplasms. The duration of treatment is about a year. The use of the immunotherapy method also prevents the recurrence of the disease in 75% of patients within 2 years.

    Beam

    X-ray irradiation of the tumor is another component of complex therapy. Using this method, it is possible to influence malignant cells localized in the bladder, the nearest lymph nodes and neighboring organs. Irradiation is carried out remotely using a special x-ray machine. In addition, the introduction of radioactive drugs into the bladder is used.     Diet

    The diet of a patient with bladder cancer needs correction. Sufficient calories must be observed, since the tumor process is often accompanied by weight loss and digestive disorders. Fractional nutrition four to five times a day, a sufficient amount of fluid (one and a half to two liters) will provide the body with nutrient requirements.

    You need to eat foods high in protein and vitamins. Of the types of heat treatment, cooking, baking are preferred. Recommended Products:

    Igor Vladislavovich, please name the risk factors for bladder cancer.

    If we talk about diseases, then the predisposing factors for the development of such cancer are chronic inflammation of the bladder and benign neoplasms: papillomas and leukoplakia. With leukoplakia, the epithelial cells lining the bladder from the inside begin to multiply rapidly, assuming an atypical lamellar form. I note that the symptoms of leukoplakia are similar to the manifestations of chronic cystitis: the same pulling pains over the bosom, frequent urination, false desires for it.

    Bladder cancer is very common: in Russia it makes up about 4% of all tumor diseases, and among malignant tumors in urology - 70%! But it is very important to emphasize that the lion's share of success in the fight against this ailment depends on the patient: will he “hear” the first warning of the disease and will use his chance to get rid of it at the stage when modern medicine allows for a confident victory.

    Tell us more about this symptom.

    Blood appears in the patient’s urine, in most cases suddenly, that is, against the background of almost complete health. The urine “turns pink” or clots are seen in it, and this can only be during one or two urinations. Blood in the urine can also suddenly disappear for a sufficiently long time, which reassures the patient.

    And while there is no pain or other signs?

    The appearance of pain after the first “bloody anxiety” is a later symptom that indicates that the tumor has grown from the epithelial lining of the bladder, where it occurs (stage I), into the muscle membrane of the bladder (stage II), and even spread to in the form of metastases beyond its borders to nearby inguinal lymph nodes (stage III) or distant organs: liver, bones and lungs (stage IV).

    The first two stages of this cancer are collectively called localized, and the third and fourth stages are common.Bladder cancer can be completely cured in stage I in% of patients, in stage II - in 60-70%. But common cancer is in less than 30% of patients. That is why it is so important to consult a urologist immediately after the appearance of blood in the urine!

    If bladder cancer so “honestly" warns of its arrival, then the results of the fight against this disease should be very encouraging?

    In the most developed countries it is. There 70-80% of patients go to the doctor at stage I of the process, and only 20-30% - at stages II-IV. In Russia, alas, the opposite is true: in contrast to residents of other countries, elderly Russians are unforgivably indifferent to this warning signal or simply do not notice it.

    Maybe our people lack medical education?

    This reason undoubtedly matters. But, I think, there is a more important one - a low standard of living, which is why many patients, especially elderly rural residents, do not have modern sanitary ware, that is, toilets. Against the background of the white color of the toilet bowls, the blood immediately becomes noticeable. On the other hand, urban old people living in comfortable apartments, due to visual impairment, may also not notice these "signals" of the disease ...

    In addition, having noticed blood in the urine, many patients hesitate to go to the urologist because of false shame and fear of examining the bladder with a cystoscope. A modern cystoscope (at the same time it has tweezers at the end for picking up the smallest portion of tumor tissue) is a non-traumatic thin conductor based on fiber optics.

    In addition, before inserting such a gentle instrument into the urethra, it is prepared by introducing special painkillers, emollients and disinfectants. Therefore, the cystoscopy procedure is now painless and safe, and if necessary, it can be performed under general anesthesia.

    Can't do without cystoscopy?

    Unfortunately not. By the way, it is also necessary to undergo an ultrasound of the bladder and other pelvic organs and pass a urinalysis. But it’s important to know about the certain limits of the possibilities of these two studies in developing bladder cancer. Ultrasound reveals a tumor with a size of at least 0.5 cm. And it is at the time when blood appears in the urine for the first time, usually it is smaller. When analyzing urine, traces of blood (and especially cancer cells) are often absent, since these symptoms appear at the beginning of a tumor with insidious inconstancy.

    So some more research is needed? What is included in the “gold standard” for the early diagnosis of this cancer?

    In addition to cystoscopy, urinalysis, and ultrasound, a microscopic examination of the tissue (histological examination) taken from the tumor and other suspicious areas of the bladder is required. It is necessary for the diagnosis of not only bladder cancer (after all, there are several of its cellular varieties that require various treatments!), But also benign tumors. If a histological examination has not been carried out, then the diagnosis of any neoplasm of the bladder, even if a tumor is visible with cystoscopy, is only preliminary!

    What to do to a patient whose blood in the urine appears again and again, but all examinations of urogenital (genitourinary) organs do not reveal an ailment?

    Such a situation is not uncommon. Previously, I had to wait for the appearance of additional symptoms. Now you can undergo photodynamic cystoscopy. A special luminous dye, accumulated only by tumor cells, is intraperitoneally injected into the patient. Therefore, with cystoscopy by a characteristic glow, you can see a tumor of such a small size that it is impossible to detect in other ways. This method is currently used in specialized urological departments and centers, such as, for example, our Institute of Urology.

    The traditional question: how is bladder cancer treated?

    If the tumor has not yet grown into the muscle wall of the bladder, then only a section of the bladder affected by the tumor is excised with a special instrument - a resectoscope inserted through the urethra (resection). But with advanced cancer, the entire bladder (cystectomy) must be removed through an abdominal incision. At the same time, inguinal lymph nodes are removed, and in men, the prostate gland is also removed.

    Features of the "nature" of bladder cancer is its extraordinary persistence. In 90% of cases, after resection of one part of the bladder, the tumor develops again, but from another part of the epithelium.

    90 percent. But this is a huge risk to wait for metastases! Why then cystectomy is not done about the smallest tumor?

    There are two reasons for this. Firstly, removal of the bladder along with the prostate is a complex and traumatic operation. And a very dangerous starlet for patients. In addition, the quality of life with a new bladder “cross-linked” from the intestinal wall decreases.

    Secondly, now there are effective drugs that are necessarily used in combination with surgery. And if we talk about stage I - localized stages of cancer, they reduce the risk of its recurrence to a minimum.

    Of the cytostatics, that is, drugs that suppress cell division, the BCG vaccine, mitamycin, doxorubicin, cisplatin are used. The most effective BCG vaccine containing weakened tuberculosis bacteria. The risk of infection with tuberculosis is excluded, but local inflammation occurs in the bladder wall, and it is quite specific, not the same as with "ordinary" cystitis. As a result, epithelial cells begin to secrete substances that have a detrimental effect on the tumor.

    Medicines (all of them are on the federal preferential list) - are introduced into the bladder through a catheter, after surgery. This procedure is carried out only in medical institutions!

    And can traditional medicine help in the fight against this ailment?

    Herbal treatment is useful in the postoperative period for rehabilitation and early recovery of the patient. Herbs have not only a diuretic, but also anti-inflammatory, hemostatic, wound healing effect. Therefore, we do not just recommend diuretic herbal preparations to patients, but prescribe them as necessary components of therapy after surgical treatment. I’ll clarify that one package of diuretic collection weighing 100 g, designed for 15 days of admission, is in pharmacies.

    What herbs are better to use?

    It is desirable that the collection includes corn stigmas, parsley seeds, St. John's wort, horsetail, wild strawberries, dandelion, chamomile, plantain. Cornflower, hernia, calendula, licorice, dill seeds, wheatgrass rhizomes, lingonberry leaves, birch (or its buds), bearberry, juniper, knotweed grass, and violet are also used.

    One collection includes 5-8 plants. Usually they are mixed in equal proportions, crushed. Then 2 tbsp. tablespoons (8-10 g) of the collection are poured with boiling water in a half-liter jar, insisted for an hour and a half and filtered. You need to drink 0.5 cups 3 times a day 30 minutes before meals for 1.5-2 months. After this, the total dose of plant material should be reduced to 1 tbsp. spoon and drink another 3-4 months, even with "good" urine tests.

    What new treatments for bladder cancer are being introduced?

    Of the most effective, I would call photodynamic therapy for stage I cancer. The team of the Research Institute of Urology of the Ministry of Health of the Russian Federation actively participated in the development of this method. Its essence is that a substance is injected into the bladder or intravenously, which is absorbed only by tumor cells. After this, the tumor cells become very sensitive to radiation. Then, a fiber guide is inserted into the bladder through the urethra on a flexible conductor and the mucous membrane is irradiated with red laser light. In this case, only cancer cells die.

    So far, this method has been used in Russia, except for our Institute, only at the Moscow Research Institute of Oncology named after P.A. Herzen. But now we are working hard to ensure that photodynamic therapy can be replicated in the oncourological departments of many hospitals and clinics in our country.

    The choice of treatment and long-term outcome (prognosis) for people with bladder cancer depends on the stage and degree of cancer. The doctor also considers his age, general health and quality of life when developing a treatment plan.

    Surgical removal of the bladder (cystectomy) is usually done at the most advanced stage of cancer, which is confined to the bladder. A cystectomy can also be done on high-grade bladder cancer or when there are multiple tumors in the bladder. Surgery cannot be recommended for an adult adult who has a long-term medical condition.

    Chemotherapy uses drugs to kill cancer cells. For early stage bladder cancer, medications can be delivered directly to the bladder using a catheter (intravesical). For the development of cancer that has deeply invaded the bladder or has spread to the lymph nodes or other organs, chemotherapy can be given orally or intravenously (IV). Side effects can be different, depending on the drugs that are used and your age and general health. For some people, depending on the stage of the cancer, chemotherapy given prior to cystectomy (neoadjuvant) leads to better results.6

    Effective treatment for bladder cancer: is there any hope of salvation?

    Before starting an effective treatment for bladder cancer, you need to contact an oncologist and undergo a complete clinical examination.

    First, it’s worth clarifying that this is an oncological disease with a high probability of a patient’s death, and a progressive pathological process spreads cancer cells to the bladder tissue.

    Such abnormal cells do not die, but, on the contrary, rapidly divide and form malignant tumors that require immediate treatment, more often, removal. Successful therapy depends on the diagnosed stage of the disease and the specifics of the body, but in this clinical picture it is advisable not to hesitate.

    General information

    The disease has a malignant nature, which means that progressive health complications may soon become the main cause of death for the patient.

    That is why it is important to be vigilant about your health and carefully monitor all its changes. For example, it is worth noting that numerous reviews of the treatment of bladder cancer report that it is very difficult and even impossible to suspect a diagnosis at an early stage due to the absence of symptoms.

    If the doctor found on the ultrasound screen a bladder tumor of the first or second stage, then we can safely count on a positive prognosis of the applied conservative therapy.

    If the diagnosis is made in the third or fourth stage, when the metastases have spread to neighboring organs, then without additional surgical intervention it certainly cannot be cured, and the final effect raises serious doubts.

    So, there are two known methods for treating bladder cancer - conservative (medication) and operative (surgical intervention with further rehabilitation), and the final choice is made by the attending physician.

    Conservative therapy

    Modern scientists have proved that conservative methods of combating malignant neoplasms also demonstrate a stable therapeutic effect, the main thing is to accurately choose the medical direction and the main drug.

    That is why doctors distinguish biological therapy as one of the most effective methods of conservative disposal of malignant tumors.

    Such immunotherapy is aimed at activating the human immune system and its opposition to cancer cells, therefore, it contains a number of medications that are most often administered from the outside through the urethra or directly into the bladder.

    The following medications are known:

    1. BCG immunostimulating bacterium: treatment of bladder cancer with its help can go into remission for a long time or, conversely, provoke another relapse. So, without medical participation, obviously can not do.
    2. Synthetic proteins of the immune system, in particular interferon, which activates the human resource in the fight against pathogenic bacteria. Interferon alfa-2b is often in harmony with BCG, and pronounced flu symptoms can become a side reaction.

    This is the main treatment for bladder cancer, which is also supplemented by chemoprophylaxis, that is, taking vitamins to reduce the number of relapses.

    But more advanced photodynamic therapy skillfully and for therapeutic purposes combines laser light and narcotic substances for large-scale extermination of cancer cells progressing in the body.

    In this case, an inactive medication is injected into a vein, which stimulates the directed flow of light and makes it destructive in relation to cancer cells. Minor areas of healthy bladder tissue can be damaged, but overall the clinical effect is excellent.

    Surgical treatment

    Typically, treatment for transitional cell carcinoma of the bladder includes surgery, radiation therapy, and chemotherapy in combination.

    The choice of procedure depends on the size of the tumor and the stage of the pathological process.

    In extensive medical practice, the following types of operations are distinguished:

    1. Transurethral resection. If the pathogenic neoplasm is relatively small, then doctors resort to transurethral resection, which consists in excising the affected areas of the mucous membrane of the bladder and further cauterizing visual defects of its structure.

    Cancer cells are destroyed by electric current (a modern laser), which is safe for the human body, and the operation to remove bladder cancer is accompanied by relatively small blood loss.

    Partial cystectomy If the pathological process progresses, but there are no metastases, the doctor may recommend a partial (segment) cystectomy, which consists in the partial removal of the bladder along with the cancer cells of its mucous membrane.

    If one area is affected, then this removal allows the patient to return to his usual life, but subject to all medical recommendations during the rehabilitation period.

    Radical cystectomy involves a deep excision of cancer cells in the affected organ, and is appropriate in the advanced stage of the disease or in the presence of metastases.

    The final excision in the latter case also affects the affected lymph nodes located in the neighborhood, and nearby internal organs. The operation is life-threatening, because there is a high risk of bleeding.

    Reconstruction of the bladder is appropriate in those case reports where complete elimination of the bladder and the creation of another way for the natural exit of urine are necessary. There are several procedures, as an option - a urostoma, the formation of a urinary reservoir and the creation of an artificial bladder.

    Chemotherapy prognosis

    Intravesical chemotherapy is appropriate both for conservative treatment of the disease and in the rehabilitation period after surgery.

    In the first case, its effectiveness is significantly lower than subcutaneous administration of BCG vaccination, but after surgery, bladder cancer and chemotherapy are compatible concepts, that is, there is a real chance of recovery (even if not final).

    So, the main goal of such a procedure harmful to the whole organism is to stop the growth, prevent the multiplication and further spread of cancer cells, and it can be achieved thanks to special medical preparations.

    After introduction into a vein or muscle, they penetrate into the general bloodstream and disperse throughout the body, affecting cancer cells on a large scale. The most effective chemotherapy for bladder cancer is through a tube inserted into the urethra.

    To achieve positive dynamics of the disease, more than one procedure is required, and the treatment course in the future will have to be renewed once every six months, while experiencing serious side effects of the body weakened by the disease.

    Chances of alternative treatment

    As you know, in alternative medicine there are also several real recipes that will allow you to destroy cancer cells and direct them to the path of recovery. For example, many patients sincerely believe in the real power of such a natural antiseptic as celandine.

    The best known treatment for celandine bladder cancer is drinking non-concentrated juice, which must be diluted with a small amount of running water.

    Single doses and the method of use should be agreed with the oncologist, otherwise you can unknowingly provoke acute poisoning of an already sick body. One thing is known: the greater the patient’s mass, the more drink he should drink per day.

    Also in folk medicine, the treatment of bladder cancer with soda is well known, which also provides improvement and the ability to return to a full life.

    The main ingredient of such therapy is ordinary baking soda, but it is advisable to find out individually the method of preparation of drinks and its proportions from a familiar oncologist. Superficial self-medication can not only worsen the general condition, but also exacerbate the prevailing clinical picture.

    • in the fourth stage, metastases spread to the prostate and abdominal walls, starting the process of metastasis.

    In most cases, the first signs appear already in the third stage of cancer, when immediate surgical intervention is indicated.

    In order to avoid surgery in the future, it is important to regularly perform a complete diagnosis of the body, at least for the purpose of prevention or for one’s mental calm. This is the only way to detect a global problem at an early stage of its appearance, and in such clinical pictures, doctors guarantee 100% recovery.
